Properly Naming the Affidavit Form:
As listed above, there are many types of affidavit forms and the type you use depends on the situation you are in or the purpose of this form. That explains the first part of writing an affidavit form which is to choose proper heading and put it on top of the paper. Although just writing affidavit at the top will clarify that it’s in fact an affidavit but the title shouldn’t be generic. It should be very close to the purpose i.e. affidavit of residence or affidavit of a stolen car.
Name the Affiant:
The person who is writing the affidavit is known as deponent or affiant. This is the part of the form where full details of affiant should be included. Starting with the full name of the person and adding his contact details, the full address where he lives along with permanent address, employment situation i.e. unemployed, businessman and personal details i.e. age, marital status, etc.
This is the middle of the affidavit form and it’s an important part because here the affiant will declare that he is writing or signing this affidavit without any pressure or force and the facts that he is providing here are completely true and he is stating these facts under oath.
Stating the Facts:
Now, this is the heart of this affidavit form because this is what the facts included in this part caused the situation where it was necessary to write and sign the affidavit. For instance, you are preparing the affidavit for your stolen vehicle so in this part; you will add the facts about the incident. You might start with a receipt from the restaurant where you were having dinner while someone stole your car. Then you will provide registration documents of your vehicle proving that in fact, you are the owner and so on.
In the end, the affiant needs to sign the affidavit form and this part should be included at the bottom of the form with some area empty around it for full name and date on which the affidavit is being signed.
